Instructions for First Visit:

**If you fill out the questionnaire prior to your appointment, you only need to arrive 10 minutes before your scheduled time.**

The first appointment is an evaluation, which is scheduled for one hour. The following appointments are 30 minutes.

A special parking rate has been arranged for our patients of $3. Please see the secretary for a "chaser ticket" to give to the parking attendant on your way out.

To get directions to Faulkner Hospital, click on the green button and then click on get directions. This will bring you to Google maps where you can plan your route and then print them out to bring with you.

The Rehabilitation department is located on the 2nd floor.

If entering the hospital from the first level, take the elevators up to the second floor, turn right out of the elevators and follow the long corridor through a set of double doors, the Rehab Department is located immediately on your left.

If entering the hospital from the third level, take the blue elevators down to the second floor. When exiting the elevators, take a left into the hallway, you will see a second set of elevators, the Rehab Department is on your right approximately 200 feet past the elevators.
