• Contact physician’s office and request Pathology Report and Corresponding Slides. 
• Have them sent to Mohs Micrographic Surgery Center, attention Administrative Coordinator, 1153 Centre Street, Jamaica Plan, MA 02130.
• If you do not receive a call within two weeks of your appointment please contact Mohs Micrographic Surgery Center at 617-983-4504 to see if slides have been received.
