How do I request imaging studies?

Imaging study requests can be made in two ways:

In Person

To request your films in person, please go to BWFH's Radiology Image Service Center, located in the outpatient registration/admitting area on the first floor (near the Emergency Department). If possible, you should print and complete the Release of Information form in advance to save time.

Mailing in a Written Request

Please include both the completed Release of Information form as well as a note stating the purpose of the request and specify what part of the medical record you need. Be sure to include your name, date of birth, social security number and the approximate date(s) of service and please do not forget to sign and date your request. Please indicate who we should send the record to and include the address, or indicate if you will be picking it up in person.

Can my child wait for me in the waiting room during my visit?

Please arrange for appropriate supervision of your child during your visit. Children under twelve must not be left unattended in the waiting area. Radiology staff are not permitted to supervise children at any time.
