*Please click visitor information to the right for directions and any other general hospital information.

Do I need a referral?

Many procedures, such as endoscopic procedures, require a referral.  Patients should check with their insurance company to be sure.

Does Faulkner Hospital accept my insurance?

Faulkner Hospital is contracted with most of the large insurance companies. Patients should check with their individual insurer.

How do I find a doctor?

Faulkner Hospital offers an online Find a Doctor directory. Please select the “Find a Doctor” tab at the top of any page to begin searching for a doctor by name, location or specialty.

How does pre registering on line help me?

Pre registering before your appointment eliminates the registration process on the date of service, allowing you to report directly to your appointment department.

What do I need to bring with me when I come for my appointment/exam?

Please be sure to bring the order that your physician gave you.  Without an order, tests cannot be performed. Your physician or the office staff may have arranged to send the order directly to the hospital department. If so, pre-registered patients can report directly to their appointment department at their scheduled date and time.

What if I don’t pre register?

While it is not required that you pre register for outpatient services, doing so eliminates a complete review of your demographic and insurance data at the time of your appointment.

Telephone pre-registration also remains an option for those patients who prefer.

The Telephone Pre-Registration Department can be reached by dialing 617-983-7010. Their hours are Monday-Friday 6 am-8 pm.

To register for Breast Imaging Services (breast MRI, bone density, mammography),call 617-983-7068. Their hours are Monday-Thursday 7 am - 8 pm, Friday 7 am - 3:30 pm and Saturday 8 am - 3 pm.

Where do I go on the day of my appointment/exam?

If you have pre-registered online or have called using our telephone services, you may go directly to your point of service. Patient Information is available at both entrances for directions.

Once at your appointment department, you will be asked to review and sign a printout of your information to verify it was entered correctly and allow us to bill your insurance company.

Who do I call if I have a question?

For pre registration questions, concerns or to complete by phone, call 617-983-7010.
For any questions regarding Breast Imaging Services, call 617-983-7068.

please see the hours of operation above for times to call between.

Who will see my personal information?

To protect your privacy, personal information is seen and accessed on a need to know basis.  Only people interacting with a particular patient will have access to the information submitted.  This would include the staff that create the pre registration, the order entry staff and the clinician.

Why doesn’t my information fill in when I return to this site?

In order to ensure our patients confidentiality, this web site does not link directly to existing Medical Records and therefore, does not fill in when you use the site again. Faulkner Hospital is committed to keeping all patient information confidential.
