Q: How do I find a primary care physician?
A: You may call our Physician Referral Line at (617) 983-7500, or you may search Physician Referral on-line or click here to request a Faulkner Hospital Physician Guide by mail.
Q: I was born at Faulkner Hospital. How do I obtain a copy of my birth certificate?
A: You may request your birth certificate from the town or city in which you lived when you were born. Or, you may request your birth certificate from the town or city where the hospital in which you were born was located. For those born at Faulkner Hospital, birth records are kept at Boston City Hall.
Q: How do I request a copy of my medical records?
A: Please visit Faulkner Hospital's Medical Records page.
Q: How do I schedule a mammogram?
  A: Please call the Faulkner-Sagoff Breast Imaging and Diagnostic Centre at (617) 983-7272 to request an appointment for a mammogram. You will need to provide information including your name, date of birth, phone number, address, and the name of the physician to whom we should send the report. If you have had films taken previously, you will be asked to bring those to your appointment. If you do not have insurance, you may be eligible for the Free Mammography Access Campaign.
